Description of Job: SnapCare is seeking a dedicated and skilled Licensed Practical Nurse (LPN) specializing in corrections care to join our healthcare team. As an LPN, you will play a crucial role in providing exceptional patient care and contributing to the overall well-being of our patients in a correctional care setting. LPN must have extensive experience in monitoring a broad range of medical conditions, recognize nursing principles and practices in providing professional nursing care under minimal supervision in a variety of settings and a thorough knowledge of nursing best practices.

Provide compassionate nursing care to patients in a post-acute care facility, ensuring their physical and emotional well-being.
Conduct comprehensive health assessments, document medical history, and collaborate on individualized care plans.
Administer medications, perform treatments, and monitor vital signs for patient safety and comfort.
Coordinate patient care by scheduling tests, consultations, and referrals.
Collaborate closely with registered nurses, physicians, and other healthcare team members.
Maintain accurate medical records and adhere to safety guidelines.
